We view Nursing Home Hospice patients as needing the same level – and continuity – of care as community patients.

Our Nursing Home Hospice patients are among our most vulnerable: they are elderly and many are dually eligible for Medicare and Medicaid.

Nursing Home Hospice Programs

Care
Our Nursing Home Hospice program increases access to comprehensive hospice care and provides appropriate care to nursing home residents who are in the final stage of life.
Comfort
In a nursing home setting, hospice helps patients, families, and nursing home staff by providing: Regular visits by a hospice Registered Nurse to the nursing home.
Compassion
Since this program began, we have been able to break the anguishing cycle of repeat ER admissions that all too often marks the end of life for nursing home residents.
Calvary Hospice

Hospice is a philosophy of end-of-life care rather than a physical place. Patients with a life-limiting illness, who are no longer pursuing treatment and want to spend their final days at peace and in comfort, are candidates for hospice care.
