Our Nursing Home Hospice patients are among our most vulnerable: they are elderly and many are dually eligible for Medicare and Medicaid.

Our Nursing Home Hospice program increases access to comprehensive hospice care and provides appropriate care to nursing home residents who are in the final stage of life.

Comfort

In a nursing home setting, hospice helps patients, families, and nursing home staff by providing: Regular visits by a hospice Registered Nurse to the nursing home.

Compassion

Since this program began, we have been able to break the anguishing cycle of repeat ER admissions that all too often marks the end of life for nursing home residents.
